Submission of this form does not guarantee a place at the College of your first choice. You will be contacted in due course for an interview. Should your application be unsuccessful with your first preference, your application will be forwarded to the College of your second choice.

To complete this application form you will need to have information about the following:

• Your enrolment details OR your UAC application preference details
• Your latest academic results (high school or university)
• Contact details of your next of kin, emergency contacts, Sydney contacts and 2 referees
• Passport number (if available)

If you do not have all of this information, you may not be able to complete the form.

You will also need to pay a non-refundable $50 Application Fee at the time you submit your application.
